OPH received Robert Wood Johnson Foundation grants to start community outreach services at Breckenridge Village, which was the early beginnings of our home health and hospice services.

A grant from the Robert Wood Johnson Foundation allowed the campus to rent a store front office in a local shopping center and offer services such as shopping, errands, transportation, companionship and information to seniors in the Willoughby area.

Subsequent grants from the Robert Wood Johnson Foundation allowed the campus to innovate new services and program to benefit the community, including the “Breakaway Club” adult day care program and Breckenridge Homecare, which paved the way for the organization’s ongoing home health and hospice services.
