Ohio Living Physician Services was created to provide patients with a team of transitional care physicians and clinicians who can provide care in their homes or via telemedicine. This proved to be an especially effective means of delivering patient care due to the arrival of the COVID-19 pandemic, when home health patients who were unable to visit their primary care physicians could schedule virtual visits via smart phone, laptop or computer without leaving the comfort of their home.
