Advancing Person-Centered Memory Care

Across Ohio Living life plan communities, including Ohio Living Swan Creek, memory care is being reimagined through a deeper commitment to compassionate, person-centered support for individuals living with memory loss.

The name reflects a deeper commitment. “Agape” represents unconditional love focused on the well-being of others, while “Crossing” honors each person’s individual journey and the meaningful ways lives intersect along the path. Together, Agape Crossing expresses a belief that those living with dementia deserve not only care, but a life filled with dignity and connection.
This philosophy is brought to life through Ohio Living’s partnership with the Hearthstone Institute and the implementation of the I’m Still Here® approach. Built on decades of research and practice, this internationally recognized model focuses on remaining abilities rather than loss.


Within Ohio Living’s Certified Memory Care Centers of Excellence, engagement is the foundation of care. Team members are extensively trained upon hire and continue their education through ongoing reviews and annual updates, equipping them with the skills to support residents more effectively.


At Ohio Living Swan Creek, this approach is especially meaningful. Daily life follows a thoughtful rhythm, or “flow,” designed to rebuild confidence and strengthen procedural memory. Through Montessori-inspired methods and evidence-based engagement programs, residents participate in experiences that feel familiar and rewarding. An inviting outdoor courtyard features comfortable seating, colorful umbrellas, and beautifully landscaped gardens. This peaceful space offers residents a welcoming setting to relax, socialize, enjoy fresh air, and connect with nature in a safe, serene environment.


“Across Ohio Living, the I’m Still Here® philosophy gives us a strong and consistent foundation,” said Jean Morgan, corporate manager of memory care at Ohio Living. “At Ohio Living Swan Creek where we have 12 memory care residents, the smaller setting allows for deeper connections and more individualized engagement. We focus on learning and conversation, while helping residents reconnect with long-term memory. That is where we see meaningful impact.”


Innovation continues to play an important role in enhancing engagement. At Ohio Living Swan Creek, tools like Lucynt, a gaming technology system, provide interactive experiences tailored to varying levels of cognitive ability, creating new opportunities for connection among residents and their families.


“This technology helps families connect with loved ones in a simple and engaging way,” explained Nancy Hamilton, director of gift planning. “It creates moments that are meaningful across generations. Whether someone is playing alongside a grandchild or enjoying a shared experience, it helps build connection in a very natural way.”
Support from the Ohio Living Foundation is pivotal to the transformation of memory care services, funding staff training, program enhancements and supplies, recertification expenses and resources that enrich daily life for residents.
